Description:

The Medical Social Consultant position provides mental health services and connections to community based resources for youth, adults, and families registered in the school based health centers of the University of Illinois Mile Square. This position also works in partnership with medical providers and other staff in the health center, with school health services, and with community agencies. This position may also serve patients that are part of the Mile Square enterprise and may participate in grant and other reporting activities.

Duties & Responsibilities:
  • Evaluate and processes referrals from medical providers, families, patients, school staff and self-referred youth who are registered in the SBHCs.
  • Diagnose and provides therapeutic interventions to treat youth, adults and families presenting with mental or behavioral health problems.
  • Function as a member of an interdisciplinary treatment team in developing and carrying out the treatment plans for youth, adults, and families.
  • Assist youth, adults, and families in accessing services through public agencies, public services, and health care facilities
  • Provide crisis intervention as needed.
  • Provide initial screening assessment for intake and emergent care.
  • Conduct comprehensive evaluation of the patient's condition, including information about current treatment, medications, social and economic factors.
  • Refers for medication consultation as indicated.
  • Interview all registered students of the school based health centers as needed.
  • Complete or contribute to an annual risk assessment for registered patients.
  • Contribute in activities related to care coordination, data collection, health information management, quality monitoring and patient education.
  • Participate in team huddle and care coordination planning and monitoring
  • Participate in approved clinical research protocols including subject recruitment, screening, and administration of research instruments in the form of questionnaires and diagnostic interviews.
  • Compose components of quality assurance and other clinical tracking activity reports.
  • Prepare and/or submit program and treatment reports using quantitative and qualitative data.
  • Perform other related duties and participate in special projects and assignments as assigned.


Minimum Qualifications Required:

  1. Master's degree from an accredited school or university in social or behavioral science or a related health specialty area.
  2. Current State of IL Licensure as a Licensed Clinical Social Worker.


Preferred Qualifications:
  • Minimum of 2 years experiences in a school and/or ambulatory primary care setting
  • Experience in child and/or family therapy
  • Experience and competence in working in a culturally diverse environment with low income patients
