Care Navigator - Outpatient Competency Restoration

Description

We are hiring for a Care Navigator to work in our Outpatient Competency Restoration/Center Point team. The purpose of the Outpatient Restoration Program is to provide competency restoration education services to individuals involved in the criminal justice system and deemed by a court to be incompetent to proceed. This is a full-time position, working 40 hours a week between Monday - Friday, 8 - 5PM, but this person must be willing and able to work as late as 7PM. This individual will be working with mostly Medicaid and Jail Based referrals. The Care Navigator will provide a minimum of 2 services a week per client. Again, these clients are individuals for whom competency has been raised in court.

Hourly Rate: $23 - $25.00 an hour

Goals and Objectives:



  • Assist with client caseloads to help increase capacity by managing inactive, and disengaged client appointments and communication
  • Communicate with care providers to improve caseload management and schedule availability
  • Administratively track and manage inactive caseloads for clinical providers


Duties and Responsibilities:



  • Extract Excel based data reports from the electronic health record on a weekly basis.
  • Organize, filter and develop tracking mechanism for interventions around caseload management.
  • Communicate with Central Access, Clinicians, Medical Providers, Supervisors and Managers regarding status of caseloads and client needs.
  • Based on the direction of the clinical team and/or AllHealth Network policies and procedures, complete administrative update and closing of client charts.
  • Schedule and cancel client appointments based case management reports and client communication.
  • Participate in telephonic communication with clients regarding their engagement in treatment. Complete and manage written communication with clients regarding patient status for receiving care at AllHealth Network. Assist with care coordination tasks such as appointment monitoring and facilitating communication with providers and other care team members.
  • Document all interventions in the electronic health record.
  • Assist BHC with caseload management for those clients identified as needing disposition as a result of disengagement clinical and non-clinical interventions and support.
  • Maintain a caseload of disengaged clients who have been identified as needing outreach. Scrub and update caseload reports on a daily basis to achieve client engagement in care or discharge from services. .
  • Perform administrative support for programs including but not limited to: medical record/chart review, documentation of patient information, review and assignment of cases to clinical staff.


  • Participate and contribute to departmental meetings, supervision and other meetings, as needed, to ensure cohesive department operations.
  • Enhance professional growth and development through participation in specialized training and other informal learning experiences.
  • Follow all AllHealth Network policies and procedures.
  • Complete all required trainings as listed in Relias Learning within required timelines.


Perform other duties as required within the scope of the position and the experience, education and ability of the employee.

Key Technical Skills and Knowledge:
Education:



  • Bachelor in Social or Human Services field



Experience:



  • One year of experience in a customer service or healthcare related field such as a hospital, home health provider, community-based agency preferred but not required.



Skills/Knowledge:



  • Possess excellent customer service skills, strong organization skills, flexibility in responding to multiple demands, able to complete routine work with minimal direction from supervisor.
  • Ability to operate PC-based software programs including high level proficiency in Word, Excel and other computer based systems.
  • Understanding or a willingness to learn care coordination and case management related service functions (i.e.: outreach and engagement, community resources, discharge planning, referrals, insurance/payer).
  • Communicate effectively in written and oral communication with health care team, clients and families.
