Responsibilities

A Transitions in Care Coordinator/Administrator ensures that new patients receive high-quality care, and that the clinic operates efficiently. New patients seen in the Peter Ho/ Infectious Disease Clinic require significant care coordination as they are enrolled in the grant funded programs. This role requires a blend of leadership, organizational skills, and healthcare knowledge. The Clinic Administrator will work closely with medical staff, support teams, and patients to maintain a well-organized and effective healthcare environment.



Qualifications

EDUCATION, EXPERIENCE, TRAINING
1. Current and valid state RN License, required
2. Current BLS (AHA/ARC) Certified upon hire and maintain current, required
3. Master of Science in Nursing (MNS) preferred
4. Knowledge of the of the following health issue: health equity, social determinants of health, sexually transmitted diseases, and HIV
5. Previous experience in healthcare management, required
6. Experience with budgeting and financial management preferred
7. Excellent interpersonal and communication skills
8. Minimum of 3-5 years' experience in a management role, required
