Title: Care Manager (RN)

Duration: 3 Months (Potential to Extend/Convert)

Location: Remote
Member Facing Face to face visits 80% work from home /Travel is based on caseload (30%)

Remote FL - Candidate must reside within region Region 7:

Orange (bilingual highly preferred), Osceola(bilingual highly preferred), Brevard, Seminole

Bilingual is preferred for any county

Shift: 8-5 M-F, no OT

Leadership works to have the new hire work within a 60 mile radius - may vary based on caseload

Must haves: Pediatric experience of 2 years minimum

Position Purpose:

The first day will align with training/onboarding. The first day will consist of setting up computer, completing onboarding tasks, reviewing training schedule, and meeting with the training team. Onboarding is 4 weeks.

  • Required to complete daily outreach calls based on member specific tiers


  • Based on members needs the call volume will vary 5-20 calls per day
  • Contractual obligation is a weighted 200 caseload - due to influx of new members the numbers are a bit skewed
  • Tier 3 - 2.2 (CW to meet with member quarterly)
  • Tier 4 - 1


  • Outreach calls are to check in on the members, follow up on their needs, remove any barriers to care for the members.
  • Full caseload 200 members (field visits are generally declined) - virtual appointments are also accepted. Some people will have 4-5 visits per week, some may have 2-3 and others (0).
  • Perform care management duties to assess, plan and coordinate all aspects of medical


and supporting services across the continuum of care for select members to promote quality, cost effective care.

  • Coordinate care (finding providers)
  • Assisting in authorization
  • Creating assessments and care plans
  • Ensuring that the children's needs are met (having issues at school, having a home health aide to assist)


Leadership will complete chart and call audits on a monthly basis or as needed

Education/Certification Required: Requires a Degree from an Accredited School of Nursing or a bachelor's degree in nursing and 2 - 4 years of related experience. Preferred: n/a
Licensure Required: RN - Registered Nurse - State Licensure and/or Compact State Licensure required" Preferred: n/a

  • care or community setting.
  • Experience connecting families/children to local resources for various services including employment, housing, independent living, justice, foster care, etc.

  • Clinical Nursing experience (Acute Care) - (pediatric)
  • Must be able to have experience working within a fast faced environment - must be able to work within stressful high volume environment
  • TEAMS, Outlook, and Excel experience (open a report and look at it, etc.)

  • Health plan experience
  • Foster care and/or adoption experience
  • Bi-Lingual
  • Knowledge of government sponsored managed care programs preferred.
  • Psych background
  • BA Case Management as a RN
  • Behavioral Health Experience
