Description
Community Outreach Mission & Overview: This is a highly autonomous, project-based Community Outreach role focused on expanding HHVI's mission throughout the community and reaching populations we have not previously served. The position supports existing and/or future outreach efforts with UH while also independently planning, organizing, and executing community-based events. The Outreach RN engages directly with community members and patients through health education and screenings-e.g. commonly blood pressure screenings-and connects individuals to hospital services as needed by coordinating appropriate referrals and follow-up care. The role typically involves coordinating and staffing a few outreach events per week, including planning and leading three annual signature events (such as Go Red, STEM initiatives, and Heart Heroes), as well as participating in large-scale community partnerships (e.g., UH Health & Safety Expo, Cleveland Browns events, and other regional activities). This position requires strong initiative and leadership-someone who can "take the bull by the horns," manage outreach logistics independently, and collaborate closely with the Director to align activities with organizational goals. The role offers significant flexibility in scheduling, including evenings and weekend events as needed. Hours can be flexed during the week to maintain work-life balance. The community outreach RN position is responsible for developing and coordinating community education and outreach. What You Will Do
