Summary:

Provide support for all Service Coordination Department functions including health education and service coordination services for CCHP members. This position also provides call issue escalation for CCHP members or providers, may manage a case load of members with low to moderate health complexity or may provide support for Team Lead duties, during the Team Lead's absence.

This position also provides call issue escalation for CCHP members or providers, may manage a case load of members with low to moderate health complexity or may provide support for Team Lead duties, during the Team Lead's absence.

Education & Experience:

  • High school diploma or equivalent required. Bachelor of Social Work preferred

  • Five years' experience in a managed care or medical office/hospital environment.

  • Bi-lingual (English/Spanish) Required. Expert proficiency in conversational, reading, writing, and translating medical terms as well as diagnoses. Must pass Spanish test prior to hire.

Knowledge, Skills & Abilities

  • Superior verbal and written communication including motivational interviewing and adult learning principles

  • Familiarity with basic medical terminology and ICD-9 and CPT coding.

  • Personal computer skills. Expertise with various software applications/databases (i.e., Microsoft Office).

  • Highly organized, ability to prioritize, and demonstrate complex problem solving skills.

  • Ability to work autonomously with minimal supervision

  • Demonstrated care coordination concepts

Licensure, Registration, and/or Certification

  • Licensed Bachelor Social Work preferred.

  • Certified Community Health Worker required.
