Job Profile Summary This role focuses on providing professional and nonprofessional nursing care services in accordance with physician orders. In addition, this role focuses on performing the following Care Manager related duties: Works as part of a multidisciplinary team to coordinate their patient's care in an effort to improve patient outcomes and create efficiencies. A management role that supervises employees focusing on tactical, operational activities within a specified area, with the majority of time spent overseeing area of responsibility, planning, prioritizing and/or directing the responsibilities of employees. Goal achievement is typically accomplished through performance of direct and/or indirect reports. A role that supervises para-professional employees. Responsibilities that typically include: Setting day-to-day operational objectives for team, problems faced may be difficult but typically are not complex, and ensures policies, practices and procedures are understood and followed by direct reports, customers and stakeholders. Job Profile Summary The Clinical Leader - RN Case Manager assesses the needs of the department and collaborates with health team members to coordinate and complete timely assessment, planning, implementation and evaluation of discharge plans and care transitions across the continuum of care. Under the supervision of the Clinical Manager/Director, this position supports the RN Case Managers in the department according to policies and procedures. Job Description Minimum Qualifications: 2. 3 Years of RN Experience with at least 1 year as a charge/lead RN. 3. Current Basic Life Support (BLS) Certification. 4. Bachelor of Science in Nursing (BSN). Preferred Qualifications: Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned. 1. Assists with the daily operational aspects of the department with respect to facilitation of work priorities. 2. Maintains professional educational development. 3. Guides the professional growth and development of RN Case Managers. 4. Participates in quality improvement activities. 5. Regularly perceives potential problem situations and intervenes to offset adverse impact. 6. Ensures departments compliance with regulatory standards. 7. Acts as a resource to staff. 8. Identifies and communicates special staffing needs and/or problems to the Clinical Manager/Director. Physical Requirements: 1. Ability to sit for extended periods of time. 2. Prolonged standing and walking. 3. Occasional twisting, bending, reaching, pushing/pulling, sitting, kneeling, and squatting. Skills & Abilities: 1. Ability to plan, implement and evaluate data and individual patient needs. 2. Ability to effectively communicate with patients, families, physicians, and healthcare team. 3. Ability to analyze, evaluate multiple solutions, and solve complex problems using well developed critical & analytical thinking skills. 4. Knowledge of and application of the nursing process. 5. Excellent time management skills needed to prioritize competing priorities. 6. Requires the ability to work independently and establish an effective working relationship with healthcare providers in physician's offices, community agencies, hospitals and other healthcare facilities. 7. Maintains working knowledge of specific benefits and reimbursement guidelines, the discharge planning process and applicable federal, state and local regulations. 8. Demonstrates proactive approach.
