Overview
The Certified Occupational Therapy Assistant provides skilled, compassionate, and goal-directed care to patients in their homes under the supervision of a licensed Occupational Therapist. This role delivers therapy services in accordance with the physician's Plan of Care, applicable regulations, and agency policies. The position supports individualized rehabilitation outcomes, fosters trust with patients and families, and advances clinical excellence across the agency's programs.
Key Responsibilities
The Certified Occupational Therapy Assistant:
* Implements therapeutic interventions that promote functional independence under the direction of the supervising Occupational Therapist.
* Collects and documents relevant data and observations to support the evaluation and Plan of Care.
* Monitors patient progress and communicates timely updates to the supervising therapist.
* Educates patients and caregivers on exercises, adaptive equipment, and safety techniques as directed.
* Maintains regular communication with patients, families, physicians, and the interdisciplinary team to support continuity of care.
* Completes documentation accurately and within required timelines.
* Participates in interdisciplinary case conferences, staff education, and quality improvement initiatives as assigned.
* Adheres to infection control procedures, safety standards, and HIPAA regulations.
* Reports changes in patient condition, incidents, or concerns promptly to the supervising therapist or clinical leadership.
* Supports onboarding and mentoring of new team members when requested.
Qualifications
* Current, active Certified Occupational Therapy Assistant license in the state of practice, in good standing.
* Minimum one year of experience as a COTA, with home health experience preferred.
* Current CPR certification.
* Strong communication and documentation skills.
* Valid driver's license, reliable transportation, and active auto insurance for patient travel.
* Ability to work independently and as part of an interdisciplinary team.
* Demonstrated commitment to patient-centered and safe care.
Work Environment
This role involves providing care in patients' homes, assisted living facilities, or clinical settings. The position requires frequent standing, walking, and occasional lifting or transferring of patients using proper body mechanics and assistive devices. Routine travel within the service area is required.
OSHA Category 1
This position involves routine exposure to blood, body fluids, communicable diseases, or other potentially infectious materials. Universal precautions, infection control procedures, and personal protective equipment are required.
