Overview
We are seeking a licensed Physical Therapist to join our clinical team. This position is responsible for delivering exceptional patient care and case management in accordance with physician orders, agency policies, and regulatory standards. The role involves developing individualized care plans, providing skilled therapy, and collaborating closely with interdisciplinary teams to support positive patient outcomes.
Key Responsibilities
* Manage a patient caseload, conducting comprehensive assessments and developing individualized, goal-oriented therapy plans.
* Provide skilled therapeutic treatments, including pain management and functional restoration through evidence-based techniques.
* Monitor patient progress, adjust care plans as needed, and communicate changes to the clinical manager and physicians.
* Coordinate interdisciplinary care, ensuring continuity between patients, families, physicians, and agency staff.
* Maintain timely, accurate clinical documentation in accordance with agency standards and regulatory requirements.
* Supervise physical therapy assistants and participate in onboarding of new staff as needed.
* Adhere to infection control protocols, safety standards, and patient rights regulations.
* Participate in quality assessment performance improvement activities and interdisciplinary team meetings.
Qualifications
* Current, active Physical Therapy license in the state of practice, in good standing.
* Minimum one year of physical therapy experience; home health experience preferred.
* Proficiency with OASIS and home health regulations strongly preferred.
* Demonstrated excellence in communication, documentation, and collaborative practice.
* Commitment to patient-centered, safe, and compassionate care.
* Ability to travel throughout the service area on a daily basis.
Work Environment
This role involves working in patient care settings, including private homes, assisted living facilities, and clinical environments. Physical activity is frequent and includes standing, walking, and occasional lifting or patient transfers. The position involves routine exposure to potentially infectious materials; adherence to infection control procedures and use of PPE is required.
