The Intake Specialist is responsible for managing the initial referral and client intake process to ensure timely, accurate, and compliant onboarding of patients/clients into care management services. The role involves verifying eligibility, collecting documentation, coordinating with internal teams and external partners, and ensuring that clients are set up for the proper care pathway according to company's standards and contract requirements. Responsibilities
- Verify client eligibility, insurance, benefits, and any required prior authorizations.
- Receive, review, and triage incoming referrals from providers, external partners, and other sources.
- Collect required documentation (demographics, medical history, consent forms, insurance cards, etc.), follow up on missing or incomplete forms.
- Enter all intake/referral data accurately into care management / case management systems (electronic health records or proprietary systems).
- Coordinate scheduling of initial appointments or service start dates.
- Communicate and liaise with providers, case managers, payers, and clients regarding intake status, missing information, and next steps.
- Monitor and track outstanding intakes, ensuring timely follow up and resolution.
- Maintain data quality, ensuring records are accurate, complete, and meet regulatory/compliance requirements (HIPAA, privacy, etc.).
- Provide reports and updates to supervisor on intake volumes, delays, and any issues or barriers.
- Participate in orientation, training, quality improvement and process optimization as needed.
