Homebase Medical is expanding rapidly into South King County - Kent, Federal Way and Auburnareas. We are currently searching for an experienced Palliative Care Nurse Practitioner OR Physician Assistant. You will be responsible for palliative care in the home setting.

Key Duties:

  • Conduct Palliative Care Visits

  • Comprehensive Assessment skills including pain management, symptom management, patient and family education.

  • Focus on goals of care.

  • Work collaboratively with our interprofessional palliative care team

  • Support after hours, holiday and weekend on-call

  • Support provider relationships with our clients

  • Actively support the achievement of Homebase Medical's Vision and Goals.

Qualifications:

  • Active Nurse Practitioner OR Physician Assistant license in the State of Washington

  • Board certification (Nurse Practitioner OR Physician Assistant)

  • DEA licensure

  • 1 year or more of palliative care experience required

  • CAPC preferred but not required

  • Experiencediagnosing and caring for Dementia/Alzheimer's patients.

  • $5,000 Sign on Bonus

  • Stable income + unlimited earning potential

  • Typical Annual Earnings: $132,000 - $212,000+

  • Guaranteed Annual Base Salary: $68,640

  • Per-patient rate: $90 for every patient seen

  • Earning security: Each pay period, you're paid whichever is higher-your base salary or your per-patient total

  • $500/month no-show protection

  • Mileage reimbursement per IRS guidelines

  • Monthly internet stipend
