QUALIFICATIONS:

Education, skills and experience:




  • High school graduate or equivalent. Associate degree preferred.
  • Minimum of 2 years previous experience working in a medical office, managed care setting, or healthcare setting.
  • Discharge planning experience including familiarity with disability process required.
  • Experience working with behavioral health patients required.
  • Florida residency required upon employment. Must live within Volusia or Flagler County.
  • Excellent communication skills, including both oral and written.
  • Strong telephone and customer service skills required. Must be able to diffuse difficult situations.
  • Strong organizational and time management skills. Must be capable of multitasking.
  • Proficiency with computer skills including Microsoft Word, Outlook, and Office Suite
  • Reliable transportation, home internet, and smart device for secondary identification required






POSITION SUMMARY:

The Non-Clinical Discharge Planner works closely with the multidisciplinary utilization management team and our members to help them achieve optimal clinical and health outcomes. Working closely with clinical staff, members and their families, the role helps to coordinate and manage the safe and efficient discharge of members from hospital to the next phase of care. This may include home care, rehabilitation or long-term care facilities. The position will also aid members in determining potential eligibility for various financial assistance programs and assisting members with finding resources to help with the application process. This may include travel to hospitals, rehab or skilled nursing facilities, and member's homes, to engage with hard-to-reach members. Additionally, the Non-Clinical Discharge Planner assists the Utilization Review Nurses and Case Managers in identifying suitable placements for those with behavioral health or long-term medical needs.

The ability to develop and maintain collaborative working relationships with FHCP colleagues and external vendors in providing case management services is essential to the role.
