Provides pharmacy services in a specialty area under the direct supervision of a licensed pharmacist/licensed independent practitioner. Demonstrates advanced technical skills and ownership for the work area. The Transplant Transitions of Care Specialist technician will work directly with the inpatient solid organ transplant team to coordinate care delivery to transplant patients within the medical center. Specific duties may include but are not limited to: medication histories and documentation within the electronic medical record, collaboration with outpatient and specialty pharmacy technicians and systems for timely processing and delivering of discharge medications, coordination of filling pillboxes at discharge with transplant patients, and assisting the ambulatory transplant clinic pharmacist.
This position is being filled for the dayshift, Monday through Friday.
