Position Overview

The Medical Social Worker (MSW) plays a vital role in supporting patients and their families by addressing the emotional, social, and financial challenges associated with illness and recovery in the home health setting. This role promotes safe care transitions, advocates for patient rights, and collaborates closely with the interdisciplinary care team to ensure holistic, patient-centered care.


Key Responsibilities
Patient Assessment & Care Planning


  • Conduct comprehensive psychosocial assessments to identify patient and family needs related to illness, recovery, and end-of-life care



  • Evaluate mental health status, insurance coverage, and available community and financial resources



  • Develop and support care and discharge plans in collaboration with the interdisciplinary team using best-practice case management principles




Interdisciplinary Collaboration


  • Communicate effectively with physicians and care team members to coordinate services



  • Participate in team meetings, case conferences, and provide on-call social work support as needed




Discharge Planning & Counseling


  • Support safe transitions from hospitals or facilities to home or alternate levels of care



  • Provide counseling, crisis intervention, advance care planning education, and bereavement support




Documentation & Regulatory Compliance


  • Maintain accurate, timely clinical documentation



  • Ensure compliance with agency policies and all state, federal, and accreditation requirements




Quality Improvement & Community Engagement


  • Participate in quality improvement initiatives



  • Support community education and outreach efforts to strengthen program growth and service reach




Patient Rights, Safety & Advocacy


  • Uphold patient confidentiality in compliance with HIPAA standards



  • Advocate for patient rights and promptly report concerns



  • Follow infection control, safety, and emergency response protocols




Professionalism & Team Support


  • Collaborate respectfully with patients, families, and colleagues



  • Support staff orientation and ongoing professional development



  • Maintain clinical competency and serve as a positive role model within the team




Administrative & Resource Coordination


  • Complete all required documentation accurately and on time



  • Coordinate services with community agencies and contracted providers



  • Manage time efficiently to meet productivity standards while delivering exceptional care




Qualifications


  • Master's degree in Social Work (MSW) from an accredited institution



  • Minimum of one year of experience in a healthcare setting; home health experience preferred



  • Strong communication, documentation, and problem-solving skills



  • Ability to work collaboratively within an interdisciplinary team



  • Demonstrated commitment to patient-centered, compassionate, and safe care
