About the Job

The Decentralized Clinical Pharmacist (DCP) at Children's Hospital of Philadelphia (CHOP) will: Provide pharmaceutical services to improve drug usage and therapeutic outcomes including, but not limited to, advising medical professionals on issues concerning drug therapy (inherent toxicity of drugs and their side effects, as well as assisting in the prescription of appropriate doses), evaluating pharmacokinetic levels, providing medication counseling and contributing to the adherence of the CHOP Formulary. Review and process medication orders using the hospital electronic health record (EHR) to ensure that the medication is appropriate in terms of dose, dosage form, route of administration, frequency, duration of therapy, and patient's disease state. Supervise pharmacy technicians and interns to ensure that pharmacy products are prepared and distributed accurately, with the appropriate quantity and at the time the product is needed. When necessary, the pharmacist prepares and distributes pharmacy product accurately, in the appropriate quantity and at the time the product is needed.

What you will do

  • Develop and maintain a consistent clinical pharmacy practice that provides optimal pharmaceutical care and support to patients of Children's Hospital of Philadelphia in an appropriate, efficacious, safe, and cost-effective fashion.
    • Participate in clinical rounds.
    • Review patient's pharmacotherapy using the hospital electronic health record (EHR) to ensure that the medications are appropriate in terms of dose, dosage form, route of administration, frequency, duration of therapy, indication and patient's disease state.
    • Communicate with prescribers and nurses to recommend alternatives for inappropriate therapy or substitutes for non-formulary items.
    • Ensure and promote compliance with institution's formulary system and other guidelines or protocols set forth by the Therapeutic Standards Committee (TSC) and the Department of Pharmacy.
    • Provide indicated pharmacokinetic monitoring to all patients on the team of clinical practice.
    • Deliver appropriate and timely consultative services (drug information, pharmacokinetics, etc.) to physicians, nurses, pharmacists and other health care professionals regarding drug therapy.
    • Provide medication discharge counseling to patients and families.
    • Ensure and maintain compliance with regulatory standards for medication management (ie Joint Commission and CMS).
  • Performs pharmacist functions to ensure accurate and timely medication delivery.
    • Responsible for compliance to all federal, state, and department standards and regulations for compounding- USP795/797/800, CMS Conditions of Participation, FDA Compounding Guidance and Joint Commission standards

  • Participate in hospital and departmental Continuous Quality Improvement (CQI) activities.
    • Utilize the Lean Principals of to analyze, and resolve problems in a systematic manner.
    • Participate in departmental LEAN Projects (RPI / 5S).
    • Work OT to allow co- workers to participate in department RPI.
    • Provide pre- and post- project feedback and support sustain measures to ensure successful outcomes.
    • Achieve patient safety and satisfaction through the use of lean principles:

    • JIT
    • Stand Work
    • Quality
    • Promote and Comply with Standardized Procedures.
    • Promote and Comply with Standard Work.
    • Identify waste in ownership process and recommend solutions for implementation.
    • Correct abnormal conditions.
  • Additional responsibilities with position evolution include, but are not limited to:
    • Experiential education of pharmacy students (IPPE and/or APPE) and residents
    • Participation in pediatric pharmacy or subspecialty focused research or education, individually or in collaboration with other pharmacy personnel or members of the multidisciplinary care team.

Licenses and Certifications

  • Licensed Pharmacist (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required

Education

  • Doctorate Pharmacy Required

Experience

  • PGY1 Pharmacy Residency Required
  • PGY1 Pharmacy Practice Residency at a Pediatric Hospital or PGY2 Pediatric Pharmacy Residency Preferred

Knowledge, Skills and Abilities

  • Extensive knowledge of drugs, medical therapeutics, pharmacokinetics, diagnosis and disease states
  • Ability to establish and maintain working relationships with various members of the health care teams
  • Ability to communicate clearly in oral and written form
  • Knowledge and comfort in using Electronic Health Records and other Pharmacy technologies
