Shift: 0730-1700, four days per week, with fairshare holiday coverage. DETAILED JOB DESCRIPTION Review all documented pressure injuries, tracheostomies, and new ostomates forinpatientpopulation. Provide skin/ostomy related consultation as ordered/needed. Perform focused skin assessment and work with theappropriate careteams/disciplines to develop and implement treatment plans per established protocol. Record, report, and interpret the patient's response to the plan of care. Perform re-evaluations as needed. Track inpatients with ostomies. Assessstomas, provide patient and caregiver education as needed, and ensure proper use of dressings, supplies, and equipment. Deliver pre-operative stoma marking, address problems with appliance adherence or peristomal skin problems, and refit as needed. Conduct post-surgical teaching for new ostomates in preparation for discharge. Work with supply companies to set uppatientfor support post-discharge. Provide individualized discharge planning, depending on the patient's medical, physical, and social needs in coordination with the care team. Discuss realistic expectations of wound healing, functional level,appropriate levelofassistancerequired by patient, andanticipatedrehabilitation progression. Develop and implement evidence-based standards of practice, with educational programs for staff and other healthcare providers.Deliver patient and caregiver education for pressure relief,appropriate positioning, skin care, and frequent skin checks. Play an integral role in tracking, treating, and preventing pressure injuries within the hospital. This includesidentifyingroot causes of such injuries and teachingstaffthe prevention of such injuries. Follow documentation of wound assessments by nursing and review charting to ensure it istimelyand thorough, supporting protocols/guidelines. Routinely review wound care outcome measures and report findings to supporting teams to evaluate program effectiveness. Provide nail trimming whendeemedneeded for quality patient care. Participate in trackingofnumber and types of consults performed by the team. Generate reports and share with clinical leadership. Participate in data collection for National Database of Nursing Quality Indicators. Participate in the education of nursing staffregardingskin care, dressing recommendations, and ostomy care. These include, but are not limited to, nurse residency programs, education modules, and other educational opportunities that arise. Educate members of different disciplines, including, but not limited to physicians, PT/OT, nurses, and dietitian students, toexpand onstaff knowledge of wound care and ostomy care. Review, trial, and educateregardingnew products to the hospital when opportunity arises. Activelyparticipatein the introduction of new products to the staff andworkswith vendors of products. May complete unit/department specific duties as outlined in department documents. REQUIRED QUALIFICATIONS Missouri Board of Nursing RN or Nurse Licensure Compact multi-state RN. Whenprimarystate of residency changes, compact state RN must apply under new state of residency within thirty (30) days. One (1) year of RN experience. PREFERRED QUALIFICATIONS Certification by the Wound, Ostomy, and Continence Nursing Certification Board (WOCNCB).Additionallicense/certification requirements asdeterminedby the hiring department. PHYSICAL DEMANDS The physical demands described here are representative of those that must be met with or without reasonable accommodation. The performance of these physical demands is an essential function of the job. This position isvery activeand requires ambulating, reaching, and moving objects weighing up to and over 20 lbs. Thisrole also routinelycomes into contact withpatients who may have contagious illnesses. This position is in a health care facility open 24 hours a day,365 daysa year.
