Description

The Infusion Clinical Pharmacist at Providence provides highquality, individualized, and costeffective pharmaceutical care while supporting safe and efficient pharmacy operations. Working closely with the healthcare team, this role ensures optimal medication therapy outcomes and enhances each patient's quality of life. As part of Providence Centralia Hospital, the pharmacist contributes to a culture of wholeperson, patientfocused care - where caregivers are valued, respected, and empowered to make a meaningful impact.

Key Responsibilities:



  • Review, verify, and validate oncology and infusion medication orders for appropriateness, dosing, interactions, and protocol compliance
  • Supervise the sterile compounding of hazardous and nonhazardous medications in accordance with USP <797>/<800> standards.
  • Collaborate with oncologists, nurses, and other clinicians to optimize treatment plans and manage supportive care
  • Monitor therapy outcomes, toxicity, lab parameters, and patient response; recommend adjustments based on clinical guidelines.
  • Provide patient education on chemotherapy regimens, side effects, safe handling, and adherence strategies.
  • Maintain and update chemotherapy protocols, order sets, and treatment pathways.
  • Ensure compliance with accreditation and regulatory bodies (e.g., USP, OSHA, Joint Commission, state board of pharmacy).
  • Support investigational drug services for oncology clinical trials when applicable.
  • Contribute to quality improvement initiatives, medication safety programs, and staff education.

Required Qualifications:



  • PharmD in Doctorate of Pharmacy, Or
  • Bachelor's Degree: Bachelor of Pharmacy
  • Upon hire: Washington Pharmacist License
  • Upon hire: Basic Life Support (BLS) (Facility Managed) - where applicable


Preferred Qualifications:



  • Acute care hospital pharmacy experience, 1 year or equivalent experience.
  • Completion of ASHP-accredited PGY1 residency.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
