BASIC PURPOSE

The purpose of the Housing Tenancy Specialist position is to assist families with sustaining permanent housing through case management, coaching and service coordination.

ESSENTIAL DUTIES AND RESPONSIBILITIES



  1. Conduct intake and program eligibility for Housing Tenancy Services
  2. Manage cases actively receiving Housing Tenancy Services
  3. Conduct case management and provide service coordination
  4. Conduct home visits based on individual housing plans
  5. Provide education on tenant roles, rights, and responsibilities
  6. Liaise between tenants and landlords to assist with issues/disputes
  7. Assist tenants with lease compliance and annual housing recertification process
  8. Conduct bi-monthly home inspections for wellness, safety, habitability checks
  9. Coordinate medical referrals in coordination with contracted managed care plans
  10. Participate in weekly case conference meetings
  11. Participate in program and agency trainings as assigned
  12. Maintain accurate participant records in various information management systems; and generate reports as requested
  13. Maintain and execute confidential information according to HIPPA standards
  14. Maintain a highly detailed and organized filling system
  15. Ensure intake procedures utilize harm reduction and housing first principles
  16. Check and respond to emails and voicemails on a regular basis
  17. Adhere to confidentiality standards
  18. Other duties as assigned


KNOWLEDGE, SKILLS, ABILITIES AND OTHER QUALIFICATIONS REQUIREMENTS



  1. Ability to speak and write the English language at a high and professional level
  2. High degree of confidentiality
  3. Able to endorse and promote The Salvation Army's mission
  4. Minimum two (2) years of social service/work and/or related experience preferred
  5. Computer literate, in Windows environment (Microsoft Office), Publisher, Excel and Outlook preferred
  6. Excellent communication skills, both written and verbal.
  7. Excellent and professional telephone etiquette and presence
  8. Excellent organizational skills
  9. Strong ability to utilize a high level of time management and handling multiple tasks


CERTIFICATES, LICENSES, REGISTRATIONS



  1. High school diploma
  2. Must possess a valid California Class C Driver License, and ability to drive a Salvation Army vehicle
  3. Must be 21 years or older
  4. Authorize The Salvation Army to add name to the CA DMV Pull Notice Program
  5. Complete The Salvation Army vehicle course training


PHYSICAL REQUIREMENTS:



  1. Ability to sit, walk, stand, bend, squat, climb, kneel, and twist on an intermittent or sometimes continuous basis
  2. Ability to grasp, push, and/or pull objects
  3. Ability to reach overhead
  4. Ability to operate telephone
  5. Ability to lift up to 25-40 lbs.
  6. Ability to operate a computer
  7. Ability to process written, visual, and/or verbal information
  8. Ability to operate basic office equipment and tools PC, Fax Machine, Telephone, Calculator, Copier, Printer.
